Job Shadow Criteria:

• Request form must be submitted at least 30 days in advance of your requested date. This ensured adequate planning and time to meet mandatory requirements.
• Proof/document of seasonal influenza vaccine is required If observing between Oct 1 -Mar 31 (or later, as directed by the Centers for Disease Control).
• Must be at least 16 years or older for all other job shadow experiences.
• Surgical shadows must be 17 years of age or older.
• Surgical experiences begin at 6 a.m. and go until the completion of the procedure (allot for a minimum of 6 hours).
