The MercyOne Health Coaches follow a unique approach to helpign patients set and achieve their health care improvement goals.

Health Coach, Angie White and Health Coach Assistant, Lori Kirkland,  are available to work with patients with chronic conditions such as diabetes, heart disease or high blood pressure to provide education and support during the journey to a healthier lifestyle.

In addition, health coaches also remind patients when it is time for their regular health screenings and wellness exams.  Our health coaches also make inpatient visits to hospitalized patients to intiate a plan to help in transition from the hospital, to home, to the clinic for a follow-up appointment with the patients provider. 

Health coaches work with patients to create a customized health care improvement plan to meet each individual's speicific needs. The plan may include eduation about specific conditions or risk factors, healthy eating tips, an exercise plan or a timeline for follow-up visits.  Most importantly, health coaches provide ongoing support to patients as they make changes to improve their health.

Health coaching has helped patients to successfully lower their hemoglobin A1C, blood pressure and lipid values to improve their health.  Patients will establish goals and steps to take during coaching sessions. High risk patients with multiple chronic conditions are great candidates for health coaching.
