Appanoose County Community Health Needs Assessment Survey Now Available
June 2, 2024MercyOne Centerville Medical Center and Appanoose County Public Health have joined forces to launch the 2025 Community Health Needs Assessment (CHNA) aimed at gathering valuable insights into the health needs and priorities of residents in Appanoose County.
The CHNA survey seeks to engage community members in a dialogue about their health concerns, experiences and goals. By understanding the unique challenges and opportunities facing our community, we can better tailor our programs and services to address the most pressing health issues.
Topics covered in the survey include access to care, community health strengths and barriers, social determinants of health and more. It is available online at https://forms.office.com/r/GAgcuYKDu1 and will be distributed through various community channels to reach as many residents as possible. The survey is available in English and Spanish. Residents can visit the Appanoose County Public Health office to take the survey in additional languages.
All community members 18 years and older are encourage to complete the CHNA survey now through July 12, 2024. Responses are confidential, and the survey takes less than 10 minutes to complete.
This assessment is a valuable public health tool to inform community members and decision makers regarding community health issues and strategies.
The results of the survey will be carefully analyzed and used to inform the development of a CHNA Report, which will be available to the public in early 2025. This report will serve as a roadmap for prioritizing health initiatives and allocating resources effectively.
