Swing Bed Program and Transitional Care
What is transitional care?
The Swing Bed Program at Skiff Medical Center is for patients not ready to go home after hospitalization, but still in need of professional nursing or therapy services. In this period of transitional care, a patient can “swing” from acute inpatient status to a transitional or rehab level of care in the same comfortable location.
Who can benefit from swing bed?
Patients in need of:
- post-hospitalization strengthening and rehab (for example, knee- and hip-replacement patients)
- wound care
- pain management
- diabetes management
- infusion therapy
- and much more
Skiff’s Swing Bed Program provides many services that make us an ideal transitional care option for you or your loved one:
- Continuity of care with the same nursing team for both acute and swing-bed patients.
- Low nurse-to-patient ratio.
- On-site physical therapy, occupational therapy, speech therapy, cardiac rehab and audiology.
- Full-service laboratory, pharmacy and respiratory therapy on site.
- The Philips Imaging Center at MercyOne Netwon, offering state-of-the-art digital MRI and low-dose CT technology.
- Close proximity to orthopedic surgeons, general surgeons, and the physicians of Newton Clinic, the MercyOne Newton Specialty Clinic and the MercyOne Newton Cancer Clinic.
- 24/7 in-house doctor coverage, with Emergency Department services available, if needed.
