Diabetes Prevention Program
Prevent T2 is a year-long lifestyle change program for people with prediabetes and those at high risk of type 2 diabetes. The program focuses on healthy eating and physical activity.
Program Benefits
This research-based program can reduce your risk of developing type 2 diabetes by up to 58% (71% for people over 60 years old).
Participant Eligibility
- 18 years or older
- A body mass index (BMI) of 25 or higher (23 or higher for people of Asian heritage)
- Have prediabetes or are at high-risk for type 2 diabetes:
- Blood test result in the prediabetes range or
- Ever had diabetes while pregnant (gestational diabetes) or
- High-risk score on the prediabetes risk test.
People who are pregnant or have been diagnosed with type 1 or type 2 diabetes are not eligible.

Program Goals
- Reduce body weight by 5% to 7%.
- Increase physical activity to 150 minutes each week.
- Increase coping skills.
What to Expect
- The class meets one time a week for the first four months. For the last eight months, the class meets 1-2 times a month.
- During classes you will work with a trained lifestyle coach and other participants to learn the skills you need to make lasting changes. You will learn to eat healthy, add physical activity to your life, mange stress, stay motivated and solve problems that can get in the way of health changes.
Get Started
- Take the risk test to find out if you are at risk for prediabetes and type 2 diabetes.
- If you are at high risk, talk to your doctor about getting a blood sugar test.
- If you have prediabetes, complete the interest form below to join a Prevent T2 program.
