Health Risk Assessments
Health Risk Assessments (HRAs) are tools for patients to get a sense of their risk for developing conditions. These self-assessments are not meant to be a clinical diagnosis or clinical review; they are meant to be directional - to provide consumers with information to discuss with their healthcare providers. The self-assessments take no longer than five minutes to complete and provide some useful information and recommendations based on how questions are answered.
How healthy is your heart?
Are you at risk for lung cancer?
Are you a candidate for medical weight loss?
Are you at risk for a stroke?
Are you at risk for type 2 diabetes?
Your hip pain - is it time to see a doctor?
Is your knee pain osteoarthritis?
