Thank you for your interest in MercyOne Dubuque Child Development Center and Preschool. Please complete the applicable forms below and submit them via email to dqcdcsch@mercyhealth.com or in person. Forms can also be mailed to:

You must submit the general enrollment forms (6 pages) and all other forms specific to your child's classroom.

• General Enrollment Forms (submit one packet per child)
• Submit Schedule Form
• Wait List Application

Health

• Health Form (submit one per child, must be signed by a medical professional)
• Health Form – Rainbow Only (submit one per school-age child, to be completed by a parent) 
• Diet Modification/Food Allergy Form (submit 1 per child with a food allergy)
• Care Plan Form (submit 1 per child with a food allergy or other health/developmental need)

Classroom Forms 
Submit one per child for their enrolling classroom

Tuition Assistance 
Submit one per eligible family

• Tuition Assistance Application Form (for full or part-time benefit-eligible MercyOne/Trinity colleagues) 
  Benefit-eligible is any MercyOne/Trinity colleague who is budgeted to work 32 hours or more per pay period.
