Child Development Center - Online Schedule Form
- Please submit form by noon on Wednesday for the following week in order to avoid late fees.
- Separate forms must be submitted for each child unless their schedules are identical.
- If you would like a submitted schedule to be a FIXED schedule, please indicate that in the notes field.
- Times must include both hour and minutes, rounded to the nearest quarter hour, and include AM or PM (for example: 7:15 AM – 12:00 PM; 8:00 AM – 5:00 PM).
