Transitional Care
MercyOne is excited to expand an impactful program which highlights relationship-centered, team-based care and serves patients and families through the most stressful part of their care journey while in MercyOne facilities. With the expansion of the Transitional Care Team (TCT), MercyOne will continue to transform the health care of our communities and act as extensions to patients’ primary care provider.
The team will include a nurse practitioner, registered nurse, certified medical assistant, and licensed practical nurse which will have the ability to collaborate with the provider and the patient enhancing their care experience with the most clinically appropriate and least restrictive levels of care. The Transitional Care Team will be alongside the patient where the patient can feel the most at home and comfortable throughout their entire circle of care ensuring the best decisions are made for the patient and approved by their provider.
“The Transitional Care Team was wonderful. We felt we had an advocate and someone on the inside and someone we truly trusted. She took time to answer our questions and explain everything. During my dad's stay we were able to call the Transitional Care Team member with any concerns and she would act immediately. She talked to staff as needed at the care facility and would email or call me back to let us know the follow-up. I was never left wondering.
