MyChart check-in

Patients with an active MyChart account can now use eCheck-In for their appointment at any of our Northeast Iowa Locations.  You can confirm your appointment via text message and check-in online through your MyChart account prior to your visit.  If you have checkrf in online, please notify the front desk you have arrived.

Check-in if you don't have MyChart

Patients without an active MyChart account should arrive at the clinic and check-in at the front desk to complete the check-in/registration process.  If you are a new patient, please arrive 15min before your appointment time and bring your photo ID, insurance card, and medication list.
