Planning Your Discharge

Our team of physicians, nurses, social workers, chaplains, dietitians and many others are available to assist you in planning follow-up care at home or at other health care facilities. Your discharge plans and instructions for continued recovery will begin the day you are admitted.

Home Care

Going home  may require extra planning, especially if you need additional care at home to aid in your recovery. MercyOne North Iowa Home Care, a Medicare certified home health agency, is a department of MercyOne North Iowa Medical Center, and is a full service agency. Professional nurses, licensed therapists, personal aides, homemakers and companions can support your care at home up to 24 hours a day.

Our home care offers home infusion therapy, enterostomal therapy and nutrition therapy. We can provide you with a hospital bed, a wheelchair, other medical equipment or oxygen services at home after your discharge. Pharmaceutical Care

During your hospitalization, all your medical needs are filled by the inpatient pharmacy. When you are ready to go home, your physician may prescribe new medications to help in your recovery. For your convenience, a MercyOne Family Pharmacy is located at MercyOne (641-428-6100) and at (641-428-5630). Your physician or nurse can call or FAX these prescriptions to the pharmacy in advance ready for you to pick-up upon discharge or you may stop and fill your prescriptions before going home.

If you live in Mason City, there is no need to wait, prescriptions can be delivered to your home at no extra cost. Refills are also available at our pharmacies with easy pick-up, delivery or mailed free to your home. If you live on the east side of Mason City, you may choose to fill your prescriptions at the MercyOne Regency Pharmacy (641-428-6940), located in the MercyOne Regency Health Plaza at 621 South Illinois, Mason City, IA. If you live south of Mason City, you may utilize MercyOne Sheffield Pharmacy (641-892-4640).

Upon Leaving

Before leaving, please check all drawers, the closet and the bathroom for personal belongings. Any medications being stored for you will be returned. You also need to retrieve any valuables, which were locked in the safe. A member of the nursing staff will accompany you from the room to your car.
